If any required immunizations are missing, a Certificate of Special Exemption (DHEC 1123) will be issued allowing the student to continue to attend school until the required immunizations can be received or their records can be transferred from another location.
The special exemption expires 30 days from the date issued and can only be issued to the student once.
If a SC Certificate of immunization has not been turned in to the school by the expiration date on the Certificate of Special Exemption (DHEC 1123), your student will not be allowed to attend school until they have received the required immunization(s) or exemption(s).
Religious Exemptions:
All South Carolina Certificate of Religious Exemption (DPH 1126) forms must be signed by a parent/guardian, notarized by a Notary Public of South Carolina , and presented to the school nurse.
